PCOC enables palliative care services to embed standardised clinical assessment tools into routine practice. PCOC captures clinically meaningful information from across palliative care patients’ disease trajectories, to measure and benchmark patient outcomes.

Essentials

Essentials is a short course for managers and clinicians learning about the PCOC assessment tools, implementing PCOC and reports.

PCOC Essentials for clinicians who need to:

  1. understand PCOC assessments
  2. refresh their knowledge of the PCOC tools.

PCOC Essentials for managers who need to:

  1. implement PCOC
  2. learn about patient outcome reports and how to use the data for change.

Fundamentals webinar

PCOC fundamentals webinar is a 1.5 hour program designed to enhance knowledge of the PCOC assessment and response framework for newly registered services and clinicians.

At the end of this webinar participants will have an improved understanding of:

  • Assessing the palliative care phase
  • Using assessment tools for symptom management, functional and performance status
  • Using assessment data to inform care and improve practice
  • Identifying strategies for integrating PCOC assessments into practice

Clinical leadership workshop: assessment and response framework

A two-hour interactive program of learning to enhance the ability to use the PCOC assessment and response framework effectively as part of routine care.

In this workshop participants will learn:

  • how to support and educate other healthcare professionals to use the PCOC assessment tools in routine practice
  • how to assess and respond clinically as informed by the PCOC assessment tools (Phase, RUG, AKPS, PCPSS, PCOC SAS)
  • strategies for integrating routine assessments into clinical practice.
Target audience

Clinical leads, senior clinicians, PCOC champions, and trainers and educators from allied health, nursing, and medicine.


Clinical leaders workshop: Using PCOC reports for quality improvement

A two-hour interactive program of learning to enhance the ability to use PCOC outcome data to drive change.

This workshop focuses on:

  • learning how to use and interpret clinical outcome data
  • diving into PCOC data to drive change
  • critically reflecting on how to use PCOC reports, including patient-level information
  • using data to target an area for improvement
  • using PCOC data to plan improvements
  • applying quality improvement strategies to plan and drive change in practice

Clinicians and managers who are familiar with PCOC reports and assessment tools


Self-directed education package

PCOC’s web-based education package can be completed in a self-directed way. The package includes presentations, useful resources and guides for assessments. These resources are available free of charge.

Target audience

Clinicians as a self-directed learning package, as well as managers, educators, and clinicians with responsibility for clinical implementation and ongoing education of the PCOC assessment and response protocol and outcome measures.

PCOC Fundamentals Workshop

An interactive, full day package designed to enhance knowledge of PCOC, the clinical assessment tools and the use of PCOC.


Target audience

Clinicians who are new users of the clinical assessment tools and / or are implementing PCOC into routine clinical practice.



PCOC Medical Officers Workshop

This one and a half hour interactive education package specifically aims to highlight the role of PCOC in palliative care services, emphasise the use of the five PCOC clinical assessment tools and provide an overview of the significance of patient outcome reports and benchmark measures.

Target audience

Medical Officers who are new users of the clinical assessment tools and / or are implementing PCOC into routine clinical practice.

This workshop is also suitable for senior members of the palliative care team who are not required to attend a full day Fundamentals Workshop, including Nurse Managers and Senior Allied Health.



PCOC Executive Officers Workshop

This one hour education package specifically aims to highlight the role of PCOC in palliative care services and highlight the significance of Patient Outcome Reports and benchmark measures.

Target audience

Senior managers and executives of palliative care services that implementing PCOC in to routine clinical practice, or are planning to do so.



Orientation Education Resources

The PCOC website has several education resources designed to introduce new users to the PCOC programme and specifically the clinical assessment tools.

The resources include:

  • A 10 minute video presentation: Orientation to PCOC assessment tools
  • A 12 minute DVD: Understanding PCOC
  • A 10 minute PowerPoint presentation with slide notes: Orientation to PCOC assessment tools.

Target audience

Clinicians who are new users of the clinical assessment tools and / or are implementing PCOC into routine clinical practice.



PCOC Clinical Manual

The Clinical Manual is designed to assist palliative care services to implement, consolidate and sustain the PCOC programme.

The manual includes:

  • Assessment tool definitions
  • Data item definitions
  • Exemplars of forms
  • Information on benchmarks and outcome measures.

Target audience

Educators and clinical support staff, managers, clinicians, IT and administration staff.



PCOC Toolkit for Palliative Care Services

An online package for the implementation, embedding and monitoring the PCOC program and assessment framework within the routine practice of a service.

The package includes:

  • A package of protocols for assessment
  • An implementation and embedding guide and self-assessment tool
  • A quality improvement audit package
  • A tool for mapping to national health standards.

Target audience

The toolkit is designed for palliative care services and clinicians. Some resources are targeted towards managers including quality improvement staff and other resources are for clinicians using the assessment framework.
