WA Framework Personal Learning Assessment

To assist in identifying your learning needs, you can complete the WA Framework personal learning assessment. The assessment form is designed for health professionals working in primary care, hospital, community, or aged care services who care for people at the end of life.

The interactive assessment form allows you to identify your knowledge gaps and development needs. You can use it to check learning needs each year or if you change your workplace or role. 

The framework looks at three levels of expertise and recognises that knowledge is a continuum that builds over time. There is a different learning assessment form for each level of expertise and it helps you identify knowledge gaps and learning needs. At each level of expertise, learning resources are mapped to the three knowledge and practice domains

To select the right Personal Learning Assessment Form, decide on your level of expertise from the below options:


Foundation level

I have no experience or very limited experience and expertise in end-of-life and palliative care.

Go to foundation level

Consolidation level

I am developing experience and expertise in end-of-life and palliative care.

Go to consolidation level

Extension level

I have significant experience and expertise in end-of-life and palliative care.

Go to extension level

What to do after completing your personal learning assessment

After you have completed your learning assessment, you will have a list of suggested options to help you meet your specific learning needs. You can use these to crease a personal learning plan.

The learning assessment and learning plan can be used to discuss your knowledge, skills and confidence in end-of-life and palliative care with your manager.

Each learning plan has room for three learning activities. You can download and complete more if needed. Remember to save the learning plan and update it as you complete your learning activities. You can then add this to your CPD portfolio and/or use it to share with your organisation.
