We are excited to announce that we have received further funding from the Australian Government, Department of Health (until the end of June 2020) to extend and expand the Advance Project. This will include ongoing support for General Practice Nurses, as well as complimentary training for General Practitioners, Practice Managers and other clinicians to enable a team based approach to initiating advance care planning and palliative care in primary and chronic/complex care.
The title of the new project is “The Advance Project: Initiating palliative care and advance care planning through training and resources for primary and chronic/complex care clinicians”. A media release about the new project is now available.
