As an inherently palliative concept, "advance care planning" is an integral part of the palliative filter and can not, for technical reasons, be searched in combination with it. You have instead a choice of a maximally sensitive search (retrieves all references on the topic of advance care planning) or a specific search (retrieves only references with a palliative care focus).
