Practice Points
Involving the GP
It is important to commence preparation for a case conference well in advance.
Decide who should attend based on the topics for discussion and need for decision making, and check staff rosters
Contact the family to discuss who should represent them and arrange a suitable time
Where a GP is needed, ask practice staff to include the case conference in the GP’s schedule (this may be in the form of an appointment in the GP’s diary, and call in by teleconference)
