What is caring@home ?

caring@home logo caring@home  aims to increase access to quality and timely end-of-life care for patients who choose to be cared for, and die at home, if possible. It develops and makes nationally available practical, training, and evidence-based clinical resources and education for health professionals to support:

  • proactive planning for end-of-life care
  • families and carers to help manage end-of-life symptoms at home through, for example, subcutaneous medicines
  • patients to be cared for in the place of their choice.

It also provides resources specific to carers and families, with a focus on Aboriginal and Torres Strait Islander families.

caring@home is funded by the Australian Government as one of the National Palliative Care Projects and is led by the Brisbane South Palliative Care Collaborative. In the new funding period (2023-2026), the project will develop new resources and education activities based on an existing primary care End-of-Life Care Framework, as recommended by the Royal Australian College of General Practitioners (RACGP) and the Australian College of Rural and Remote Medicine (ACRRM) for community-based patients.
