GPs delivering community-based palliative care

caring@home offers three free education activities for GPs:

  • Supporting end-of-life planning and care
  • End-of-life planning mini audit
  • FastTrack CPD.

The Supporting end-of-life planning and care webinar explores the key role of GPs in delivering high quality, person centred palliative and end of life care at home. It introduces the Prompts for End-of-Life Planning (PELP) Framework to support early identification, care planning and goal based conversations, demonstrates the palliMEDS app for managing terminal phase symptoms, outlines the Core Medicines List for timely access to essential palliative medicines, and highlights the use of clinical audit to improve practice in line with RACGP standards and national guidelines.

The end-of-life planning mini audit course is a practical, reflective learning activity designed to support GPs in delivering high quality, compassionate end of life care. Participants review care provided to three patients with a life limiting illness, using current evidence based guidance and the Prompts for End-of-Life Planning (PELP) Framework to reflect on identification, planning, documentation, and care delivery. The activity encourages structured reflection, identification of improvement opportunities, and optional peer discussion to support continuous quality improvement and meaningful CPD aligned with everyday general practice.

Four core medicines are commonly used to manage terminal symptoms. The two FastTrack CPD activities help GPs delivering home-based palliative care understand these drugs, their appropriate use, and legal considerations such as the doctrine of double effect. Both activities include a clinical fact sheet and a multiple-choice-question quiz. The focus of these activities is:

  • FastTrack CPD 1 - Terminal care: management of pain and respiratory secretions
  • FastTrack CPD 2 - Terminal care: management of anxiety and emesis.

  • End-of-life planning mini audit course: 30 minutes of EA CPD and 2.5 hours of MO CPD
  • Each FastTrack CPD activity: 30 minutes of EA CPD and 30 minutes of RP CPD.

The Supporting end-of-life planning and care and End-of-life planning: mini audit webinars are formally accredited with RACGP and ACRRM.
