This presentation outlines the Decision Assist project. Decision Assist is a federally funded project, managed by a consortium of national health and aged care organisations. The Decision Assist project was initiated in part because of an ageing population that is accessing aged care facilities later and for less time.
 
The Project is designed to:

  • Provide specialist palliative care and advance care planning advice
  • Improve linkages between aged care services and palliative care services
  • Enhance palliative care skills and advance care planning expertise
  • Improve the quality of care for aged care recipients, prevent unnecessary hospital admissions and shorten hospital stays

Palliative Care Linkages Project
 
The Palliative Care Linkages Project is a subset of the Decision Assist project and is designed to enhance linkages between palliative care services and aged care providers. Evidence points to there being five strategies that promote linkages between organisations. These are:

  • Multidisciplinary team structures,
  • communication pathways,
  • designated linkage person,
  • role clarification and knowledge exchange and
  • upskilling.

Twenty projects are funded across Australia to implement linkage enhancement projects.
