The Gippsland Regional Palliative Care Consortium (GRPCC) commissioned the development of this model to:
  • Help community palliative care services meet the challenge of increasing demand; and
  • Support capacity building.
The model is based on Palliative Care Australia’s Standards for Providing Quality Palliative Care for all Australians (2005) and is informed by the linked report. It also draws on the policies and procedures of Melbourne City Mission Palliative Care. The model is intended to complement the policies and procedures of individual organisations.
