End-of-life care in residential aged care facilities

This website has been created for you in response to a three-phase pilot project developed by the Brisbane South PHN and Metro South Palliative Care Service to enhance your delivery of end-of-life care within residential aged care facilities. 

Phase one concentrated on determining residents’ values, wishes and preferences (131kb pdf) while the second phase focussed on best clinical practice at end of life (180kb pdf).  The focus of phase three is on quality end-of-life care using the End-of-Life Care Clinical Model (115kb pdf) aiming for improving resident, family and staff satisfaction with care and increased numbers of residents dying in their place of choice.
