Two case studies – Case management of AIDS dementia clients in the community

OBJECTIVE: To illustrate the changing face of case management with complex clients who have no real personal support and are dependent on health professionals.

METHOD: Two in depth case studies will be described. One study will describe a client with a structured case management programme. All support was given by health professionals. There are multiple health professionals located at least 6 different sites involved with this client. The other case study concerns an AIDS Dementia client on an unstructured programme with fewer supports.

SUMMARY OF RESULTS: Funding has been allocated for the structured programme on a one off basis to allow this person to remain in the community. Behaviours have become appropriate and clients are keen to comply with the care offered in case management

CONCLUSION: That case management of AIDS Dementia clients needs to be responsive, adaptable and flexible and service delivery must be designed around the specific character and needs of the client. Co-ordination of service delivery from a range of services is the key indicator for success and regular reviews / updating are necessary for the management.
