This new project has been funded by the Australian Government until June 2016. The overall aim of
the project is to strengthen and build better health
connections between the health and aged care systems by:

•    providing specialist palliative care advice and advance care planning advice to aged care providers
     and GPs caring for people receiving aged care services
•    improving the linkages between aged care services and palliative care services
•    improving the quality of care for aged care care planning expertise of the aged care workforce and
      GPs caring for older Australians receiving aged care services
•    improving the quality of care for aged care recipients, by preventing unnecessary hospital
      admissions and shortening hospital stays.

The project outcomes will include a dedicated national hotline for GPs and aged care providers to call for access to specialist palliative care advice and information about advance care planning. It will also feature web-based resources, and additional training and linkage opportunities to help provide the best possible care.
