The impact of care giving has been well documented in the literature. Many family care givers identify that care giving can be burdensome (Halley, 2001) and lack adequate resources to cope (Nijboer, Triemsta, Sanderman & van der Bos, 1996). As such, it is not uncommon for patients to readmit to hospitals as a result of the care giver not being able to effectively manage the situation at home. The Learn Now, Live Well program has been specifically designed to better prepare care givers with relevant education and resources prior to discharge. Funded through the Caring Communities' initiative the Learn Now, Live Well project aims to increase carers' confidence in supporting their family member at home. This presentation will outline the development, implementation and initial evaluation of the project.   The program, consisting of six modules, will be offered in both community and inpatient settings. The modules topics include pain, medications, symptom control, nutrition & exercise, communication and the physical side of caring. Evaluation strategies for the program will be outlined, and progress to date.
