Advance Care Planning in Residential Aged Care

Advance Care Planning provides an opportunity for people to think, discuss and plan for the medical treatment they would prefer if they became too ill in the future to express their wishes. As part of this process people may choose to document their wishes. People are encouraged to legally appoint individual/s to act as their substitute decision-maker(s).
