Victoria has completed two pilot projects under the Department of Health and Ageing Palliative Care for People at Home initiative.
- Improving access to palliative care for people who have had a stroke and their carers. This project will establish a partnership model of care between palliative care services and stroke services
- Developing the role of a pharmacist in community palliative care multidisciplinary teams to improve outcomes for people at home, and their carers’
The Role of the Pharmacist in the Community Palliative Care Multidisciplinary Team
This project was a pilot of a model of care that involves a pharmacist providing community palliative care clients and their carers with best practice assessment, advice and evidence-based information to assist them in remaining at home.
