As an inherently palliative concept, "advance directives" is an integral part of the palliative filter and can not, for technical reasons, be searched in combination with it. You have instead a choice of a maximally sensitive search (retrieves all references on the topic of advance directives) or a specific search (retrieves only those references with a palliative care focus).

Advance Directives: Declarations by patients, made in advance of a situation in which they may be incompetent to decide about their own care, stating their treatment preferences or authorizing a third party to make decisions for them (Source: Bioethics Thesaurus). This search includes living wills.
